What to Expect


 

NEW PATIENT STEPS

 

1) Scheduling Your First Appointment

When your physician refers you to our clinic or you contact us directly, you will speak with one of our front desk new patient coordinator’s. She will assist in coordinating an appropriate date and time that works best with your schedule for your appointment with us.

 

2) Preparing for Your First Appointment

A few days before your appointment, you will receive a reminder call. On the day of your appointment, please arrive 30 minutes prior to your scheduled appointment and be sure to bring with you the following items:

  • A list of your current medications and allergies
  • A valid photo ID and insurance card
  • Reports for recent relevant imaging studies

 

3) When You Arrive

Upon arrival, please be sure to check in with the receptionist at the front desk. She will collect your medication list, photo ID/insurance cards, and imaging reports. You will then be given an iPad to complete your new patient information. If assistance is needed with navigating the iPad, we have staff ready to assist you with any questions. You will then be directed to read and sign our HIPPA agreement, appointment policy, medication agreement, and financial agreement. Once completed, please be prepared to pay your co-pay at this time.

 

4) In the Exam Room

One of our friendly medical assistants will lead you to the exam room. She will take your vitals and discuss the reason for your visit. Following your consultation with the physician, our receptionist will set you up with your next appointment.
